29 CFR 1926.405(a)(2)(ii)(I): Flexible cords and cables shall be protected from damage. Sharp corners and projections shall be avoided. Flexible cords and cables may pass through doorways or other pinch points, if protection is provided to avoid damage. a. The extension cord used to power a cutting wheel that was up on the scaffold had damage to the exterior insulation, thereby exposing employees to an electrical hazard.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. Note to paragraph (b)(2)(v): As of January 1, 1998, subpart M of this part (1926.502(d)) provides that body belts are not acceptable as part of a personal fall arrest system. The use of a body belt in a tethering system or in a restraint system is acceptable and is regulated under 1926.502(e). a. Employees were using an aerial lift for access to the scaffold affixed to the bridge support, the employee was not wearing fall protection, while in the aerial lift nor when he exited the lift over the guard rails.

29 CFR 1926.760(a)(1): Except as provided by paragraph (a)(3) of this section, each employee engaged in a steel erection activity who is on a walking/working surface with an unprotected side or edge more than 15 feet (4.6 m) above a lower level shall be protected from fall hazards by guardrail systems, safety net systems, personal fall arrest systems, positioning device systems or fall restraint systems. a. Employees engaged in work to raise the bridge were not wearing fall protection, while working between 15-16 feet in elevation, thereby exposing the employee to a fall hazard of 15 feet. Or in the alternative 29 CFR 1926.501(b)(1): "Unprotected sides and edges." Each employee on a walking/working surface (horizontal and vertical surface) with an unprotected side or edge which is 6 feet (1.8 m) or more above a lower level shall be protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a. Employees engaged in work to raise the bridge were not wearing fall protection, while working between 15-16 feet in elevation, thereby exposing the employee to a fall hazard of 15 feet.

The employee was observed climbing the shoring tower to take equipment to the work area approximately 15 feet from the ground, thereby exposing the employee to a fall hazard of 15 feet.
