29 CFR 1910.213(d)(1): Each circular crosscut table saw was not guarded by a hood which shall meet all the requirements of paragraph (c)(1) of this section for hoods for circular ripsaws: Mold shop area, center of the shop, on or about July 23, 2014, and at times prior thereto, an employee cutting plywood was exposed to an amputation hazard from an exposed table saw blade that did not have the guards attached to the Hitachi table saw.

29 CFR 1910.305(g)(2)(iii): Flexible cords and cables were not connected to devices and fittings so that strain relief is provided that will prevent pull from being directly transmitted to joints or terminal screws: Production area, cement vibrating table #3 in the center of the facility, on or about July 23, 2014, and at times prior thereto, employees were exposed to an electrical hazard while turning on the cement vibrating table #3(Make: Vibco, model: 2P-800) that did not have strain relief on the 230V/460V flexible power supply/connection cord.

29 CFR 1910.1200(f)(5)(i): The employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the identity of the hazardous chemical(s) contained therein: Production area, Northwest side of the facility, on or about July 23, 2014, and at times prior thereto, employees were exposed to a chemical hazard from a 2000 gallon tank that contained a chemical called concrete form oil (Concrete Form Release Oils, GuardForm Active Penetrating Form Release Agent, Petroleum hydrocarbon fluid) and was incorrectly affixed with the label identifying the chemical in the 2000 gallon tank as Dixie oil company, concrete form release oil.
