Permanent aisles and passageways were not appropriately marked: In the shop: On or about September 14, 2017, and at times prior thereto, employees were exposed to struck-by hazards while working the shop with no aisle markings for the operation of powered industrial vehicles.

29 CFR 1910.253(e)(6)(i): The regulator inlet connection for the pressure-reducing regulator did not comply with Regulator Connection Standards, 1958, Compressed Gas Association: In the Welding Shop: On or about September 14, 2017, and at times prior thereto, employees were exposed to fire hazards, while using an oxygen compressed gas cylinder that had a pressure gauge that was missing the glass cover and the needle was inoperable to measure pressure.
