29 CFR 1926.501(b)(13):     Each employee engaged in residential construction activities 6 feet (1.8 m)  or more above lower levels was not protected by guardrail systems, safety net systems, personal fall arrest systems, or an alternative fall protection measure under another provision of paragraph 1926.501 (b), nor did the employer demonstrate that it is infeasible or creates a greater hazard to use these systems and develop and implement a fall protection plan which meets the requirements of paragraph (k) of section 1926.502:      Worksite located at 401 W. 10th Street, Fort Calhoun, NE:  Employees were observed conducting construction activities on a residential structure.  There were three employees and owner who were exposed falls of 9 feet or more without using any form of fall protection such as a personal fall arrest system.       Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(1):     Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder.      Worksite located at 401 W. 10th Street, Fort Calhoun, NE:  An extension ladder used for access and egress to roof did not extend at least 3 feet above the surface being accessed.  The ladder was also not secured at its top to a rigid support.       Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.

29 CFR 1926.1053(b)(4):     Ladder(s) were used for purposes other than the purpose for which they were designed:      Worksite located at 401 W. 10th Street, Fort Calhoun, NE:  The top section of an extension ladder was being utilized to access a roof surface.  The ladder is not designed to be used in separate sections as outlined in the labeling on the ladder side rail.       Abatement certification is required for this violation.  The abatement certification sheet is enclosed with the citations.
