29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  a)  Armor Roofing, LLC on or about July 24, 2014, at the address of 35897 Wyndemere St. Avon, OH: The employer failed to ensure employees were protected from falls while removing old roofing material from a top a residential type structure.  Employees working in this condition were exposed to falls up to 18 ft. to the ground below.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  a) Armor Roofing, LLC on or about July 24, 2014, at the address of 35897 Wyndemere St. Avon, OH: The employer did not ensure that employees had been trained in the appropriate use of fall protection such as but not limited to the use of personal fall arrest systems or alternative measures while performing residential roofing operations.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  a)  Armor Roofing, LLC on or about July 24, 2014, at the address of 35897 Wyndemere St. Avon, OH: The employer did not ensure the side rails of the aluminum extension ladder were extended at least three feet above the landing/working level. Employees utilizing the ladder in this condition were exposed to falls of approximately 10 ft. while using the ladder to access the working level.
