29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):  (a)  Armor Roofing, LLC on or about March 10, 2015, at the address of 36741 Chester Rd, Avon, OH: The employer did not have a plan or program ensuring frequent and regular inspections of the job site are performed by a competent person.  Such inspections would provide identification and correction of common job site hazards such as falls exposures during job site operations, decreasing the chance of employee injury and/or illness.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) Armor Roofing, LLC on or about March 10, 2015, at the address of 36741 Chester Rd, Avon, OH: The employer failed to ensure employees were protected from falls while installing new roofing material on a commercial type structure.  Employees working in this condition were exposed falls of 11 ft. fall to the ground below.   Armor Roofing, LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 986474, citation number 1, item number 1a and was affirmed as a final order on August 14, 2014, with respect to a workplace located at 35897 Wyndemere Avon, OH.

29 CFR 1926.503(b)(1): The employer did not verify compliance with paragraph (a) of this section by preparing a written (training) certification record including the name or other identity of the employee trained, the date(s) of the training, and the signature of the person who conducted the training or the signature of the employer.  a) Armor Roofing, LLC on or about March 10, 2015, at the address of 36741 Chester Rd, Avon, OH: The employer did not develop, maintain or produce upon request a written record of certification for all employees who might be exposed to fall hazards. It had been evaluated during employee and employer interviews that exposed employees had been trained, though the employer did not produce a written certification of their training.
