Air Ground Xpress, Inc., 12712 Route 66, Clarion, PA 16214, storage container structure, on or about July 24, 2014 and times prior thereto:  Electrical equipment including, but not limited to, a Peak Blue DEF pump and lighting, located in a storage container structure was powered via a spliced flexible extension cord that was wired to the storage container structure.  The flexible extension cord and the equipment within the structure was powered via a 120 volt electrical receptacle.    B.)  Air Ground Xpress, Inc., 12712 Route 66, Clarion, PA 16214, yellow post near diesel tank / pump, on or about July 24, 2014 and times prior thereto:  A set of lights on a pole were powered via a spliced flexible extension cord.  The source for the flexible extension cord and the lighting was a 120 volt receptacle on the pole.    C.)  Air Ground Xpress, Inc., 12712 Route 66, Clarion, PA 16214, north side of the main building, on or about July 24, 2014 and times prior thereto:  An electrical junction box was equipped with a cover plate that was not properly affixed to the junction box.
