29 CFR 1910.1018(e)(2): The employer, having a workplace or work operation covered by this standard did not monitor the workplace and work operation to accurately determine the airborne concentration of inorganic arsenic to which employees may be exposed. On or about 29 September 2014, employees in the ion implant area were using arsenic to refurbish semi conductor parts without the employer conducting initial air monitoring for arsenic, preventing accurate determination of the airborne levels of arsenic emitted by the process; exposing employees to an inhalation hazard.

29 CFR 1910.305(e)(1) Cabinets, cutout boxes, fittings, boxes, and panelboard enclosures in damp or wet locations were not installed so as to prevent moisture or water from entering and accumulating within the enclosures. The enclosures were not weatherproof in wet locations. On or about 29 July 2014, employees were using type 1 electrical units that were not rated for moist and/or damp environments to power production equipment that was located in the chemline area, exposing employees to electrical hazards of ~240 volts.
