29 CFR 1903.19(d)(1): The employer must submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  a.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, December 31, 2014, for a violation of 29 CFR 1910.212(a)(3)(ii), issued as citation 1, item 2, and described as follows:  At the workplace, the point of operation of a slitter, machine 110, was not adequately guarded, exposing employees to an amputation hazard.  At the workplace, the point of operation of the laminator / buildup machine was not adequately guarded, exposing employees to an amputation hazard.  b.  The employer has failed to submit abatement certification and documentation within ten days of the abatement date, January 30, 2015, for a violation of 29 CFR 1910.219(f)(1)(i), issued as citation 1, item 3, and described as follows:    a.  At the workplace, the laminator / buildup machine, had a gear that was not adequately guarded, thereby exposed employees to a caught-in hazard.

29 CFR 1910.212(a)(1): Types of guarding. One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc.    a.  At the workplace, the S & S Flexographic machine 146, on the operator side, near the printer stations, had rotating rollers that were not guarded, thereby exposing employees to an ingoing nip point hazard.

The guarding device shall be in conformity with any appropriate standards therefor, or, in the absence of applicable specific standards, shall be so designed and constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle.    a. 29 CFR 1904.29(b)(1): What do I need to do to complete the OSHA 300 Log? You must enter information about your business at the top of the OSHA 300 Log, enter a one or two line description for each recordable injury or illness, and summarize this information on the OSHA 300-A at the end of the year.  Located at Jet Container Company in Columbus, Ohio:  a. On the 2014 OSHA 300 Log the employer did not provide all information required in Column F such as the object that directly caused the injury.  The Jet Container Company was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 313157000, citation number 1, item numbers 1a through 1d and was affirmed as a final order on June 25, 2010 with respect to a workplace located at 2050 South High Street, Columbus, Ohio 43207 .
