29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable: 29 CFR 1910.134(c)(1)(v): Procedures and schedules for cleaning, disinfecting, storing, inspecting, repairing, discarding, and otherwise maintaining respirators: a. At the work site, an employee had a tight-fitting half-face respirator provided by the employer. The respirator was visibly soiled and hanging from a hook on the basket of the aerial lift while he used a hand grinder on mortar and bricks.

29 CFR 1910.134(c)(1): In any workplace where respirators are necessary to protect the health of the employee or whenever respirators are required by the employer, the employer shall establish and implement a written respiratory protection program with worksite-specific procedures. The program shall be updated as necessary to reflect those changes in workplace conditions that affect respirator use. The employer shall include in the program the following provisions of this section, as applicable: 29 CFR 1910.134(c)(1)(viii): Training of employees in the proper use of respirators, including putting on and removing them, any limitations on their use, and their maintenance: a. At the work site, an employee had a tight-fitting half-face respirator provided by the employer that was visibly soiled and hanging from a hook on the basket of the aerial lift while he used a hand grinder on mortar and bricks. The employer had provided inadequate training for the employee regarding the care and maintenance of the tight-fitting half-face respirator.

29 CFR 1926.453(b)(2)(v): A body belt shall be worn and a lanyard attached to the boom or basket when working from an aerial lift. a. At the time of inspection on July 29, 2014, an employee was observed wearing a body harness that was not tethered to the aerial lift basket. The employee was approximately thirty feet above the ground. Quality Masonry was previously cited for a violation of this Occupational Safety and Health standard,29 CFR 1926.453(b)(2)(v), which was contained in OSHA inspection number 975547, citation number 1, item number 1 and was affirmed as final order with the informal settlement agreement on July 2, 2014, with respect to the workplace located at the exterior of the United States Post Office in Cambridge, Ohio.
