29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person: On or about 07/25/2014, at the job site at 11712 Beaverton Drive, Bridgeton, MO, employees engaged in roofing operations were exposed to hazardous conditions created by the employers failure to conduct frequent or regular inspections. The employees were exposed to an approximate 10 foot fall hazard while working on the roof without wearing a harness that had been properly tied off to an anchor point. Also, they were exposed to struck-by hazards as a result of using a nail gun without required eye protection.
