29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: A). On or about 07/29/14, at the site of 160-68 Willets Point Blvd. Whitestone, NY Employees working underneath of scaffolds were in danger of head injury from falling objects such as, but not limited to tools and pallets of bricks from other employees who working on above section of the scaffolds. Protective helmets were not worn. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19

On or about 07/29/14, scaffolds at 160-68 Willets Point Blvd. Flushing, NY Employee working on the fabricated frame scaffolds in the back of the building was exposed to the hazard of falling approximately 12 ft. to the ground. The working platforms were installed with only two or three scaffold planks. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface was not used: A). On or about 07/29/14, at 160-68 Willets Point Blvd. Flushing, NY Employees were ascending/descending a scaffold by climbing the frame and were in danger of falling approximately 12 ft. to the ground. An access ladder or equivalent safe devices were not provided. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19.
