On the roof where employees were sealing drywall inside where the HVAC systems will be located, the plywood cover over the vent was not secured from displacement thereby exposing employees to a fall hazard.    On October 19, 2016 an employee stepped onto the plywood when not secured and the employee fell four floors into the duct work severely injuring the employee.
