29 CFR 1910.184(e)(1): Alloy steel sling(s) did not have permanently affixed durable identification stating size, grade, rated capacity, and reach:    a) The employer does not require alloy steel slings to have identification stating size, grade, rated capacity and reach.  This violation was most recently observed at 275 North Queen Street, Shippensburg on July 31st, 2014, where alloy steel slings located in the Large Blast Area did not have identification stating size, grade, rated capacity, and reach.  To abate this ongoing violation, the employer must provide and require that all alloy steel chains are equipped with permanently affixed durable identification stating size, grade, rated capacity, and reach.    b) The employer does not require alloy steel slings to have identification stating size, grade, rated capacity and reach.  This violation was most recently observed at 275 North Queen Street, Shippensburg on July 31st, 2014, where alloy steel slings located in the Small Blast Area did not have identification stating size, grade, rated capacity, and reach.  To abate this ongoing violation, the employer must provide and require that all alloy steel chains are equipped with permanently affixed durable identification stating size, grade, rated capacity, and reach.      c) The employer does not require alloy steel slings to have identification stating size, grade, rated capacity and reach.  This violation was most recently observed at 275 North Queen Street, Shippensburg on July 31st, 2014, where alloy steel slings located in the Osborn Shakeout Area did not have identification stating size, grade, rated capacity, and reach.  To abate this ongoing violation, the employer must provide and require that all alloy steel chains are equipped with permanently affixed durable identification stating size, grade, rated capacity, and reach.      d) The employer does not require alloy steel slings to have identification stating size, grade, rated capacity and reach.  This violation was most recently observed at 275 North Queen Street, Shippensburg on July 31st, 2014, where alloy steel slings located in the SPO Shakeout Area did not have identification stating size, grade, rated capacity, and reach.  To abate this ongoing violation, the employer must provide and require that all alloy steel chains are equipped with permanently affixed durable identification stating size, grade, rated capacity, and reach.      e) The employer does not require alloy steel slings to have identification stating size, grade, rated capacity and reach.  This violation was most recently observed at 275 North Queen Street, Shippensburg on July 31st, 2014, where alloy steel slings used to secure ladles to the forklift while being transferred from furnace to pour deck did not have identification stating size, grade, rated capacity, and reach.  To abate this ongoing violation, the employer must provide and require that all alloy steel chains are equipped with permanently affixed durable identification stating size, grade, rated capacity, and reach.    Domestic Casting Company LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 924719, citation number 2, item number 2 and was affirmed as a final order on 03/28/2014, with respect to a workplace located at 275 North Queen Street, Shippensburg.    Domestic Casting Company LLC was previously cited for a violation of this occupational safety and health standard, which was contained in OSHA inspection number 116215120, citation number 1, item number 5 and was affirmed as a final order on 06/01/2011, with respect to a workplace located at 275 North Queen Street, Shippensburg.    Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.219(m)(1)(i): Standard conditions were not secured by the use of the following materials:  expanded metal, perforated or solid sheet metal, wire mesh on a frame of angle iron, or iron pipe securely fastened to floor or to frame of machine:  a) The Tower - Employees doing maintenance on the wet elevator were exposed to a guard that was not securely fastened to the floor or to the frame of the machine, on or about July 31st, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
