29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):    (a)  Porch Roof - Worker was observed working on roof without using fall protection and was exposed to a 10 feet fall hazard on or about July 31, 2014.   (b)  Rooftop - Workers were observed performing  roofing work on a steep roof without using fall protection and were exposed to a 42 feet 9 inches fall hazard on or about July 31, 2014.   (c)  Front of  House Rooftop - Workers were observed working off of a steep roof without using fall protection and were exposed to a an approximately 38 feet fall hazard on or about July 31, 2014.   (d)  Second Floor - Workers were working around an unprotected floor opening without using fall protection and were exposed to a 9 feet 10 inches fall hazard on or about July 31, 2014.   (e)  Third Floor - Workers were using a stairway landing that was missing its guardrails  without using fall protection and were exposed to a 9 feet 10 inches fall hazard on or about July 31, 2014.   Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed:  (a)  Front of House  - Workers were using an extension ladder that was approximately 2 feet above the upper landing surface to access the rooftop, on or about July 31, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
