29 CFR 1926.651(b)(1): The estimated location of underground utility installations, such as sewer, telephone, fuel, electric, water lines, or any other underground installations that reasonably may be expected to be encountered during excavation work, were not determined prior to opening an excavation: (a) Construction Site: On or about July 24, 2014, and at times prior, employees were exposed to fire and explosion while digging an approximately forty-eight inch deep hole with a Bobcat S13 Skid Steer Loader when installing a pole barn.
