OSH ACT of 1970 Section (5)(a)(1):  The employer did not furnish employment and a place of employment which was free from recognized hazards that were causing or likely to cause death or serious physical harm to employees in that employees were exposed to struck by hazards from falling stock.  a)  Warehouse;  On or about July 21, 2014 and at times prior thereto, the employer failed to ensure that  at least three (3) free standing damaged storage racks whose structural integrity had become compromised were repaired or that section(s) of racks taken out of service until they could be repaired which exposed employees to the hazards associated with getting struck-by falling product and sections of rack.  b)  Storage racks throughout the warehouse;  On or about July 21, 2014 and at times prior thereto, the employer exposed his employees to struck-by hazards by falling product and sections of rack in that the employer failed to ensure that all column base plates of storage racks were anchored to the floor with anchor bolts, only half the racks at the facility were bolted to the floor.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:    a)  LFA Cross Member #1;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to potential hazardous energies (including but not limited to electrical, pneumatic, and gas) in that the employer permitted employees to perform servicing and maintenance on equipment without having a written machine specific lock-out tag-out procedure to ensure the energy sources would be controlled.    b)  LFA Cross Member #2;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to potential hazardous energies (including but not limited to electrical, pneumatic, and gas) in that the employer permitted employees to perform servicing and maintenance on equipment without having a written machine specific lock-out tag-out procedure to ensure the energy sources would be controlled.    Saehaesung Alabama, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.147(c)(4)(i), which was contained in OSHA inspection number 315111799, citation number 2, item number 1 and was affirmed as a final order on April 6, 2012, with respect to a workplace located at 210 Progress Drive, Andalusia AL 36421.

29 CFR 1910.255(b)(4): All press welding machine operations, where there was a possibility of the operator's fingers being under the point of operation, were not effectively guarded by use of a device such as an electronic eye safety circuit, two hand control, or similar protection:    a)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #1 Chowell spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    b)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #2 spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    c)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #3 spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    d)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #4 spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    e)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #6 Chowell spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    f)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #7 Chowell spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    g)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #9 spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    h)  Nut and Bolt line;  On or about July 21, 2014 and at times prior thereto, the employer exposed employees to crush-by/amputation hazards in that employees were allowed to operate the #10 Chowell spot welder where the point of operation was not provided with any type guarding such as an electronic eye safety circuit, two hand control, or other similar protection as required by the regulation.    Saehaesung Alabama, Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.255(b)(4), which was contained in OSHA inspection number 315846360, citation number 1, item number 1 and was affirmed as a final order on February 22, 2012, with respect to a workplace located at 210 Progress Drive, Andalusia AL 36421.
