On or about July 31, 2014, and at times prior thereto, Rig #23, the leading edge of the stairway landing on the northwest side of the mud pits was sticking up approximately 2 ½ inches above the mud pit floor. This condition exposed employees to a trip and fall hazard.

29 CFR 1910.303(b)(1)(iv): Electric equipment was not free from recognized hazards that were likely to cause serious physical harm to employees, such as damaged electrical insulation:    On or about July 31, 2014, and at times prior thereto, Rig #23, the outer insulation of the 220V cable power lead connected to the main transformer was pulled approximately 3 to 4 inches away from the metal clamp. Black electrical tape was used to repair the damage but did not duplicate the cord's original characteristics; neither the jacket's strength nor flexibility characteristics were restored. Employees were exposed to electrical shock hazards.
