OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees such as struck-by or overhead crushing hazards.    a) On and before August 4, 2014, in the second floor attic storage area, employees were exposed to an overhead crushing hazard while working to drain water lines and  two 55 gallon drums, used to collect rain water, that are located below a cracked and broken horizontal roof support beam supported by two temporary 4 inch by 4 inch by 1/8 inch thick angle iron columns.      b) On and before August 4, 2014, in the grid department, plastisolve dipping oven area, and the cutting department, employees were exposed to an overhead crushing hazard while working below a second story wooden attic floor which was temporarily supporting a cracked and broken horizontal roof support beam with two 4 inch by 4 inch by 1/8 inch thick angle iron columns.

On or about August 4, 2014, at workstation number nine in the burring department, the employer exposed employees to an electrical shock hazard by allowing employees to use and work near a flexible cord, which powered an overhead electrical hoist, that was not fitted with a strain relief device at the plug end thus exposing the wires under the insulation.
