29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets: A). On or about 08/08/14, at the site of 2-25 147th Street. Whitestone, NY Employees working underneath of scaffolds were in danger of head injury from falling objects such as, but not limited to tools and pallets of bricks from other employees who working on above section of the scaffolds. Protective helmets were not worn. ABATEMENT VERIFICATION IS NOT REQUIRED PURSUANT TO 29 CFR 1903.19

29 CFR 1926.452(j)(3): When guardrails were used for fall protections, a workbench meeting the requirements was not in use: A). On or about 08/08/14, at the site of 2-25 147th Street. Whitestone, NY Employees working on a frame scaffold were exposed to the hazard of falling approximately 15 ft. to the ground. The top rails of the scaffolds were missed. ABATEMENT VERIFICATION IS REQUIRED PURSUANT TO 29 CFR 1903.19
