29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems: a. Site- An employee installing fire suppression equipment on top of the canopy over the gas pumps was exposed to a fall of approximately 18 feet from the top of the parapet wall when getting on and off of the scissor lift being used to access the work area. b. Site- An employee installing fire suppression equipment on top of the canopy over the gas pumps was exposed to a fall of approximately 15 feet where the parapet wall around the top of the canopy was only 34 inches tall.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided: a. Site- An employee installing fire suppression equipment on top of the canopy over the gas pumps was exposed to a slip and trip hazard where a ladder or stair was not provided to get up and down from the 34 inch parapet wall that was being used for access to the top of the canopy from a scissor lift.
