29 CFR  1904.39(a)(2):Basic Requirement. Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA:  Location:  3020 Nichols Avenue, Trumbull, Connecticut  On or about March 20, 2023, the employer did not report to OSHA within 24 hours that an employee was hospitalized due to a workplace incident where an employee slipped off a truck, fell backward on the concrete floor, and was hospitalized due to head injuries.

29 CFR  1904.41(a)(1):29 CFR 1904.41(a)(1) If your establishment had 250 or more employees at any time during the previous calendar year, and this part requires your establishment to keep records, then you must electronically submit information from the three recordkeeping forms that you keep under this part (OSHA Form 300A Summary of Work-Related Injuries and Illnesses, OSHA Form 300 Log of Work-Related Injuries and Illnesses, and OSHA Form 301 Injury and Illness Incident Report) to OSHA or OSHA's designee. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms:  Location: 35 Philmack Drive, Middletown, Connecticut:     On or about March 31, 2023, the employer having 489 employees and operating under NAICS Code  238220, had not electronically submitted their OSHA 300A Annual Summary data for calendar year 2022 which was required by March 2, 2023.
