29 CFR 1910.146(c)(5)(ii)(F): Employees entered into permit spaces that met the conditions set forth in 29 CFR 1910.146(c)(5)(i) but the atmosphere within space was not periodically tested to ensure that the continuous forced air ventilation was preventing the accumulation  of a hazardous atmosphere. Any employee who enters the space or that employee's authorized representative, shall be provided with an opportunity to observe the periodic testing required by this paragraph:                                       At this jobsite, in one of the containers being assembled, three employees were welding inside a nearly completed container where an entrance cover was removed exposing employees to hazards from falling out of the opening and getting caught-in, struck-by, or crushed in the opening if the barrel continued to move, or moved in a direction where an employee could lose footing and fall out.
