29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks: On or about 10/12/17, and at times before, the employer failed to ensure that the lathes, including, but not limited to the Warner & Swasey M-1740, No. 5 Lathe, Lot No.55, Serial No. 595155, had the following installed: 1) A chuck guard, and 2) A guard to protect against flying chips and sparks near the tooling.

29 CFR 1910.178(a)(6): The employer did not ensure that all nameplates and markings were in place: On or about 10/12/17, and at times before, the employer failed to ensure that the forklifts, including but not limited to the Yale lift truck, model #GLP035VXNUSE084, serial #C810V01721C, had control markings in place. Two of the control levers were missing markings that indicate the operations that the levers perform.
