The Crane Trust located approximately 6611 Whooping Crane Drive, Wood River, NE: The employer is failing to ensure that unsafe equipment and/or machinery are tagged and/or locked to render inoperable, or physically remove from service. The employer did not have the loader taken out of service to repair a quick coupler pin; required to keep attachments attached to the front boom.

29 CFR 1926.21(b)(2): The employer shall instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. The Crane Trust 6611 Whooping Crane Drive, Wood River, NE: The employer is failing to ensure that employees are trained and/or instructed in the recognition and avoidance of unsafe conditions as well as the applicable regulations to their work environment in that: A. The employer has not instructed employees to not work near/under suspended loads B. The employer did not ensure that all employees using a Caterpillar 924G were affectively and/or adequately trained in the operational limitations and safe operation of the equipment and its attachments. On or about August 1, 2014, an employee was assisting to construct a corral for a bison habitat. The employee was holding a level to the corner post while his supervisor used the bucket of a Caterpillar Loader model 924G / PIN# CAT0924GJDDA01421, filled with dirt; to drive the post in to the ground; placing the employee in a hazardous location underneath the bucket of the loader. As the post was being driven in to the ground the bucket came off of the loader's coupler; causing the bucket to fall and empty on the employee; resulting in severe injuries.
