29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level was not protected from falling to that lower level: Employees at a commercial job at 6633 W. Kellogg in Wichita, Kansas were exposed to fall hazards of 15 feet. Two employees applying sheeting on the portico from a work platform positioned on the forks of the JLG Gradall G6-42P telehandler were not protected from falling in that the front side of the work platform did not have a guardrail.

29 CFR 1926.451(c)(2)(v): Fork-lifts were used to support scaffold platforms while the entire platform was not attached to the fork and the fork-lift was moved horizontally while the platform was occupied: Employees at a commercial job at 6633 W. Kellogg in Wichita, Kansas were exposed to fall hazards of 15 feet. Two employees applying sheeting on the portico from a work platform positioned on the forks of the JLG Gradall G6-42P telehandler were exposed to falling to the concrete below in that the 4' by 12' work platform was not fully seated onto the forks and it was not secured to the mast or forks of the telehandler.
