29 CFR 1903.19(d)(1): The employer did not submit to the Agency, along with the information on abatement certification required by paragraph (c)(3) of this section, documents demonstrating that abatement is complete for each willful or repeat violation and for any serious violation for which the Agency indicates in the citation that such abatement documentation is required:  (a)    The employer NC Construction, LLC failed to provide documentation demonstrating the abatement methods and completion as cited in Citation, 1 and Citation 2 for hazards associated with; 1-1) electrical hazards while operating a Makita Circular Saw with damage to the outer insulation exposing the inner conductors; 1-2) A Krause MultiMatic extension ladder did not extend past the upper landing surface 36"; 2-1)  employees sheating and building walls on new residential structure were not using any type of conventional fall protection system, employees exposed to fall hazard of 12'1".

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:  A Krause MultiMatic brand extension ladder was used to access to the upper landing surface did not extended past the upper landing surface 36", allowing employees a hand hold while ascending and/or descending.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:    Employees were sheathing and building walls on a new residential structure while not using any type of fall protection system. Potential fall height was measured to 12' 1".  The NC Construction LLC was previously cited for a violation of this occupational safety and health standard or its equivalent standard CFR 29 1926.501(b)(13), which was contained in OSHA inspection number 975867, citation number 1, item number 2 and was affirmed as a final order on 6/27/14, with respect to a workplace located 204 Raydo Circle, Springfield, OH.
