29 CFR 1926.1053(b)(1): Where (a) portable ladder(s) was/were used for access to an upper level landing surface and the ladder's length did not allow the ladder side rails to extend at least 3 feet (0.9 m) above the upper landing surface being accessed, the ladder was not secured at its top to a rigid support that will not deflect, and/or a grasping device was not provided to assist employees in mounting and dismounting the ladder. a) For employees working at 1200 Quail Drive in Sarasota, as observed on or about August 08, 2014. Employees were observed using an extension ladder to gain access to the roof. The eave to ground height was approximately 35 feet. The ladder was extended above the upper landing surface approximately 12 inches.

Employees were observed working on the roof of a new residential home where ten 32 inch tall reinforcing steel bars were unguarded approximately 28 feet 8 inches beneath the working location.

Employees were observed working on the roof with a 5:12 pitch. The eave to ground height was approximately 35 feet and the employees had no fall protection system in place. Rejevaz Services Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 973834, citation number 01, item number 001 and was affirmed as a final order on June 30, 2014, with respect to a workplace located at Alistar Drive, Wimauma, Florida.
