29 CFR 1926.20(b)(2): The employer did not initiate and maintain programs which provided for frequent and regular inspections of the job site, materials and equipment to be made by a competent person(s):    a. During construction work at 708 S. Main St., St. Charles, MO, on August 8, 2014, where fall protection was not required by the employer during roofing work on the 7:12 and 5:12 roof slopes and approximately 10 ft. 2 in. and 10 ft. 7 in. ground to eave heights, and where the foreman was working on the roof also without fall protection, an inspection was not made by a representative of the employer who was aware of the fall protection requirements, and who prohibited employees from working in an unsafe condition.    Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.  The abatement certification sheet is enclosed with the citation(s).

During construction work at 708 S. Main St., St. Charles, MO, on August 8, 2014, where fall protection was not required by the employer during roofing work on the 7:12 and 5:12 roof slopes and approximately 10 ft. 2 in. and 10 ft. 7 in. ground to eave heights, and where the foreman was working on the roof also without fall protection, site specific fall protection training was not provided to ensure work complied with the fall protection requirements.  Abatement certification and abatement documentation is required for this violation.  The documentation should include written verification of abatement, applicable measurements or monitoring results, and photographs or videos which you believe will be helpful.
