29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: (a)Carlos Perez Construction at 5296 Andes St., Denver, CO 80249: On and before August 9th, 2014, the exposing employer did not ensure that the path to ground was in place for the yellow extension cord, used with a Porter Cable sawzall, model 747, serial number 165211A 5031. The condition exposed employees to 120 volt electrical shock hazards.

The condition exposed employees to 120 volt electrical shock hazards.
