29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system. On August 12, 2014, employees were working on a roof on a residential home under construction without using proper fall protection. This condition exposed employees to falls greater than 6 feet. CEBALLOS ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313499022, CITATION NUMBER 001, ITEM NUMBER 01 AND WAS AFFIRMED AS A FINAL ORDER ON NOVEMBER 17 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 334 EMERALD DR. HORIZON TX, 79938. CEBALLOS ROOFING WAS PREVIOUSLY CITED FOR A VIOLATION OF THIS OCCUPATIONAL SAFETY AND HEALTH STANDARD OR ITS EQUIVALENT STANDARD 29 CFR 1926.501(B)(13), WHICH WAS CONTAINED IN OSHA INSPECTION NUMBER 313498966, CITATION NUMBER 001, ITEM NUMBER 01 AND WAS AFFIRMED AS A FINAL ORDER ON MAY 22, 2011, WITH RESPECT TO A WORKPLACE LOCATED AT 6480 GEYSER, EL PASO, TX, 79912.

29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed. On August 12, 2014, employees were using a ladder that did not extend at least 3 feet above the upper landing. This condition exposed employee to fall hazards greater than 6 feet.
