29 CFR 1910.176(a): Aisles and passageways were not kept clear and in good repair with no obstruction across or in aisles that could create a hazard:    (a) Machine Shop, Punch Press Areas - Metal carts containing window components obstructed the aisleway that ran between the metal storage racks that separated the two work cells that made up the Machine Shop.  These metal carts made it more difficult for employees operating powered industrial trucks to maneuver to get dies or store dies in the metal storage racks, exposing employees to hazards including, but not limited to, tipping of the powered industrial truck and getting struck by a dropped or falling load, on or about August 5, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.176(b): Storage of material created a hazard:    (a) Machine Shop, Punch Press Areas - The employer stored dies on metal storage racks that ran along the sides of the Punch Press Areas.  Employees used powered industrial trucks to pick dies off the racks and to store dies on the racks.  The employees operating the powered industrial trucks picked and stored the dies from the sides of the racks opposite the sides where machine/press operators worked.  The racks did not have anything to prevent dies from getting knocked off the racks during picking and storing operations, and dies have gotten knocked off the racks, falling into the area where machine/press operators worked, exposing the machine/press operators to the hazard of getting struck by falling dies, on or about August 5, 2014.    Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.217(c)(2)(i)(a): Point of operation guards on mechanical power presses did not prevent entry of hands or fingers into the point of operation by reaching through, over, under or around the guards:    (a) Machine Shop, Punch Press Area, Double Hung Side - The fixed barrier guard attached to the die, tagged SK111, on the A 3 1/2 Niagara 45-ton full revolution mechanical power press, serial number 37154, used to punch metal window pans, had openings in the front measured at 8 inches, 11.5 inches, and 7.5 inches long by 4 inches to 4.5 inches high; openings on the top at each side measured at 11.5 inches long by 3 inches wide, and openings on both sides measured at 5.5 inches long by 3.5 inches high, which permitted entry of the hands or fingers into the point of operation by reaching over and under the guard, on or about August 5, 2014.    (b) Machine Shop, Punch Press Areas - The employer used numerous dies on numerous full revolution mechanical power presses actuated by foot pedals.  Most of the dies had at least two stations for punching, forming, shaping, or cutting window components.  The machine operator would hand-feed a single window component into one station within a die at a time; however, when the machine operator activated the foot pedal, the ram of the press would cause all stations within the die to cycle, exposing the machine operator's hands or fingers to the other points of operation within the die, on or about August 12, 2014.     Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.

29 CFR 1910.1200(f)(5):  Except as provided in paragraphs (f)(6) and (f)(7) of this section, the employer did not ensure that each container of hazardous chemicals in the workplace was labeled, tagged or marked with the following information:  (i) Identity of the hazardous chemicals contained therein; and, (ii) Appropriate hazard warnings, or alternatively, words, pictures, symbols, or combination thereof, which provide at least general information regarding the hazards of the chemicals, and which, in conjunction with the other information immediately available to employees under the hazard communication program, will provide employees with the specific information regarding the physical and health hazards of the hazardous chemical:  (a) Machine Shop, Punch Press Area, Single/Double Hung Side -The blue 55-gallon drum used to store a mixture consisting of oil and kerosene, used as a die lubricant, had no label, tag, or marking indicating the identity and appropriate hazard warnings for the hazardous chemical contained within the 55-gallon drum, exposing employees to injurious contact with the lubricant, on or about August 12, 2014.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and state employees and their representatives were informed of this abatement.
