29 CFR 1910.212(a)(3)(ii): The point of operation machine guards were not designed or constructed as to prevent the operator from having any part of his body in the danger zone during the operating cycle:    a) Saw Room; Saw #2 - Employees were not protected from rotating saw blades. The Lexan plexi-glass fixed barrier guards present on the equipment contains a large opening where employees can reach their hand through and/or around the guard and into the danger zone during the operating cycle. An employee was injured, on or about March 16, 2015 when the employees hand came into contact with the rotating saw blade while retrieving material.    Abatement certification and documentation required within 10 days after abatement date.  The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement.  Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.

29 CFR 1904.39(a)(2): The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours:    a) 1551 Mt. Rose Avenue, York PA - The employer failed to report the in-patient hospitalization of an employee to OSHA within twenty-four hours for an incident occurring on or about March 16, 2015.    No abatement certification or documentation required.
