29 CFR 1926.304(i)(1): Each circular had-fed ripsaw was not guarded by a hood which completely enclosed that portion of the saw above the table and that portion of the saw above the material being cut. On or about August 13, 2014 an employee was exposed to an amputation hazard while using a table saw without the blade guard. Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).

29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: On August 13, 2014 an employee was not utilizing a personal fall arrest system while working from a ladder jack scaffold, exposing the employee to a fall of 16'6" to grade. Stas Construction Corp. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(1)(i) which was contained in OSHA inspection number 958317, citation number 01, item number 01 and was affirmed as a final order on March 11, 2014, with respect to a workplace located at 211 Arlington, Elmhurst, Illinois. No abatement documentation or certification is required of this item.
