29 CFR 1910.178(l)(3)(ii)(H): 29 CFR 1910.178(l)(3)(ii)(H): Powered industrial truck operators did not receive training in the hazards of closed environments and other areas where insufficient ventilation or poor vehicle maintenance could cause a buildup of carbon monoxide: Ba-Tampte Pickles Product Inc. Employee drove and operated Hyster forklift inside the "wet room". As a consequence there was a build up of Carbon Monoxide -900 ppm according to FDNY report- Employees working in the "wet room" became ill due to exposure to carbon monoxide in the sealed windowless room. The employer did not ensure that the employees receive training in the hazards of close environments when operating powered vehicles. The condition was noted on or about August 13, 2014. NOTE: IN ADDITION TO ABATEMENT CERTIFICATION, THE EMPLOYER IS REQUIRED TO SUBMIT ABATEMENT DOCUMENTATION FOR THIS ITEM, FAILURE TO COMPLY WILL RESULT IN AN ADDITIONAL PENALTY OF $1,000.00 AS PER 29 CFR 1903.19.
