Balconies were not barricaded with guardrails and no fall protection was being used.   a) On or about 08/08/2014 employees were observed to be exposed to fall hazards of 20 feet while standing on a balcony ledge.  b) On or about 08/13/2014 employees were exposed to fall hazard of 30 ft. while unloading material from forklifts off of balconies.

Scott St., Tampa, FL 33602, on or about 08/08/2014, employees of Benson Construction Services, LLC were working from a trash bucket that was secured to the forks of a rough terrain forklift and using it as a manlift. The forklift was operated by Siltek Group Inc., the general contractor for the site. The bucket was not designed to be used as a manlift and was not designed to be used with the forklift in this manner.
