29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports. The employer does not ensure that each platform on all working levels of scaffolds is fully planked or decked between the front uprights and the guardrail supports. This violation was observed on or about August 14, 2014 when an employee was exposed to a fall hazard while performing masonry work from the second level of a scaffold without it being fully planked. The Pedro Velazquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1)(i), which was contained in OSHA inspection number 962976, citation number 1, item number 2a and was affirmed as a final order on May 20, 2014, with respect to a workplace located at 213 Dove Meadow Dr. Conroe, TX 77384. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that all working levels of scaffolds are fully planked or decked between the front uprights and the guardrail supports.

29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used. Cross braces were used as a means of access. The employer does not ensure that employees have a ladder, stairway, or ramp to access a scaffold. This violation occurred on or about August 14, 2014 when an employee was exposed to a fall hazard while performing masonry work from the second level of a scaffold without adequate access. The Pedro Velazquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(e)(1), which was contained in OSHA inspection number 962976, citation number 1, item number 3 and was affirmed as a final order on May 20, 2014, with respect to a workplace located at 213 Dove Meadow Dr. Conroe, TX 77384. The employer does not protect employees on scaffolds from falling when working more than 10 feet above lower levels. This violation was observed on or about August 14, 2014, at the job site where an employee was exposed to an approximate 11 foot fall hazard when working from a scaffold platform without being protected from falling. The Pedro Velazquez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(g)(4)(iv), which was contained in OSHA inspection number 962976, citation number 1, item number 4 and was affirmed as a final order on May 20, 2014, with respect to a workplace located at 213 Dove Meadow Dr. Conroe, TX 77384. Pursuant to 29 C.F.R. 1903.19, within ten (10) calendar days of the abatement date, the employer must submit documentation showing that it is in compliance with the standard, including describing the steps that it is taking to ensure that employees are protected from falling when working on scaffolds 10 feet or more above lower levels.
