29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. a) On or about August 12, 2014, at the worksite located at 531 Opportunity Way, Lagrange OH, two employees were exposed to a fall hazard when they were setting metal piggy back trusses at a height of approximately 23.9 ft. high and not tied off. b) On or about August 12, 2014, at the worksite located at 531 Opportunity Way, Lagrange OH, one employee was exposed to a fall hazard of approximately 23.9 ft. high, while climbing in and out of the powered mobile scaffolding, during the setting of metal piggy back trusses. c) On or about August 12, 2014, at the worksite located at 531 Opportunity Way, Lagrange OH, two employees were exposed to a fall hazard when they were walking the metal piggy back trusses, during installation, at a height of approximately 23.9 ft. high and not tied off.
