29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs: On or about Aug 13, 2014, an employee was exposed to a fall hazard approximately 10 feet in height to unpaved ground level below from a scaffold, which had only 2 out of 3 possible planks installed while employee was installing trim boards around a window frame. Jose Alvarado was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.451(b)(1), which was contained in OSHA inspection number 978570 , citation number 1, item number 2 and was affirmed as a final order on July 10, 2014, with respect to a workplace located at 18910 Real Ridge, San Antonio, TX. 78256.
