Maintenance Shop, Shelikof Street: On or about 12 August 2014 and times prior thereto, the blue, floor mounted bench grinder work rest measured approximately 3/8 inch from the abrasive wheel. This condition exposed employees to a struck by hazard. APL, Limited was previously cited for a violation of this occupational safety and health standard 29 CFR 1917.151(g)(4), which was contained in OSHA inspection number 891268, citation number 1, item number 5 and was affirmed as a final order on 06/10/2013, with respect to a workplace located at Terminal One, Dutch Harbor, Alaska.
