29 CFR 1917.152(e)(2)(iv): Exposed metal parts of welding cable lugs were not insulated: Maintenance Shop: On or about August 12, 2014 and at times prior thereto, the wire feed cable housing on the Miller 24 A, 24 Volt Wire Feed Welder was cracked, removed, and repaired with tape that was less than the insulating capacity of the cable housing connection. This condition exposed the operator to the conductors.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: Maintenance Shop: On or about August 12, 2014 and at times prior thereto, where hazardous chemicals were used in the workplace including cleaners (Napa Battery Terminal Cleaner, 3M High Power Brake Cleaner) and lubricants (Chevron Delo 400, Air Tool Lubricants) the employer could not provide a current written hazard communication program. This condition exposed employees to inadequate chemical information regarding the use of hazardous chemicals in their workplace.

29 CFR 1917.44(o)(5)(i): The employer did not provide, or make available in the terminal's service area a chart containing instructions and information provided in current United States Department of Transportation, National Highway Traffic Safety Administration (NHTSA) publications, entitled "Safety Precautions for Mounting and Demounting Tube-Type Truck/Bus Tires"� and "Multi-Piece Rim/Wheel Matching Chart,"� or equivalent publications pertinent to the type(s) of multi-piece rim wheels being serviced: Maintenance Shop: On or about August 12, 2014 and at times prior thereto, the employer did not have the necessary poster when servicing multi-piece rims of the 10-100-20 and the 11R-22.5 type. This condition exposed employees to struck-by hazards in the event improper wheel components are utilized to make multi-piece rim repairs.
