29 CFR 1926.451(g)(1): Each employee on a scaffold more than 10 feet above a lower level were not protected from falling to that lower level.  On or about August 14, 2014 and times prior thereto, at the workplace located at or near 11819 Maumelle Blvd, North Little Rock, Arkansas 72113, employees engaged in construction framing from scaffolding at least 10 feet above the ground below did not have fall protection. This condition exposed employees to the hazard of fall from an elevation.

This exposed employees to the hazard of impalement.

29 CFR 1926.1051(a): Stairway(s) or ladder(s) were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, or no ramp, runway, sloped embankment, or personnel hoist was provided:  On or about August 14, 2014 and times prior thereto, at the worksite located at or near 11819 Maumelle Blvd, North Little Rock, Arkansas, employees performing construction framing climbed from the top of the work area to the ground below without use of a stairway or ladder. This condition exposed employees to the hazard of falls from an elevation greater than ten feet.
