What this inspection record means

OSHA opens inspections for many reasons: routine scheduling under a national or local emphasis program, an employee complaint or referral, or a follow-up after a reported injury. Opening or conducting an inspection is not itself an allegation or a finding that this employer broke any rule; any findings appear as the citations listed below, and citations can be contested, reduced, or withdrawn.

Initial $2,800 · Current $2,800
29 CFR 1926.1053(b)(1): When portable ladders are used for access to an upper landing surface, the ladder side rails shall extend at least 3 feet (.9 m) above the upper landing surface to which the ladder is used to gain access; or, when such an extension is not possible because of the ladder's length, then the ladder shall be secured at its top to a rigid support that will not deflect, and a grasping device, such as a grabrail, shall be provided to assist employees in mounting and dismounting the ladder. In no case shall the extension be such that ladder deflection under a load would, by itself, cause the ladder to slip off its support. On or about September 9, 2014, the employer did not ensure the aluminum extension ladder extended three feet above the roof landing. The employees used the ladder to access the roof while performing roofing activities of a 1 story, commercial structure. This condition exposed the employees to the hazard of stumbling and falling from the ladder 12 feet to the ground below.
