29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees: a) At the work site, on 08/18/2014, near the south corner Navigation Boulevard at Lockwood Drive in Houston, Texas, employees were exposed to engulfment hazards while preparing to install a traffic light foundation section inside a 4'8" deep trench which was not equipped with a safe means of egress.

29 CFR 1926.651(j)(2): Protection was not provided by placing and keeping excavated or other materials or equipment at least 2 feet (.61m) from the edge of excavations, or by the use of retaining devices that were sufficient to prevent materials or equipment from falling or rolling into excavations, or by a combination of both if necessary: a) At the work site, on 08/18/2014, near the south corner Navigation Boulevard at Lockwood Drive in Houston, Texas, employees installing traffic signal base foundations were exposed to struck-by hazards due to spoil piles and concrete debris not being kept at least 2 feet from the edge of the trench area.

29 CFR 1926.651(k)(1): Daily inspections of excavations, the adjacent areas, and protective systems were not made by a competent person for evidence of a situation that could have resulted in possible cave-ins, indications of failure of protective systems: a) At the site, where the competent person inspecting the excavation was not competent in that they could not determine a safe means of egress from the excavation, the safe position of spoil piles, determine the type of soil which made up the area and was not familiar with requirements of 29 CFR Subpart P.
