29 CFR 1926.451(b)(1)(ii): The platform was not planked or decked as fully as possible and the remaining open space between the platform and the uprights exceeded 9.5 inches (24.1 cm):    a) At the jobsite, on or about August 19, 2014 - A Carpenter was working on the platform of an approximately 22 foot high scaffold which was not fully planked constructing an overhang.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     a) At the jobsite, on or about August 19, 2014 -  Carpenters engaged in roofing activities on a 4/12 pitch roof and a back porch with fall hazards of approximately 25 and 17 feet respectively were not protected from falling to the lower levels.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:    a) At the jobsite, on or about August 19, 2014 - The employer did not provide employees exposed to fall hazards who were working on a scaffold, roof edge and back porches with fall protection training.  The fall hazards were approximately 22, 25 and 17 feet respectively.
