29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Jobsite, 100 Arrowhead Point, Slippery Rock, PA: On or about November 27, 2019 the portable ladder being used to access the upper work area did not extend above the landing surface at least 3 feet nor was a grab bar installed to assist employees when using the ladder. The fall hazard from the access point on the upper surface is approximately 22 feet.
