29 CFR 1926.250(a)(1): Materials stored in tiers were not stacked, racked, blocked, interlocked, or otherwise secured to prevent sliding, falling or collapse:  On or about June 11, 2014, and at times prior thereto, workers pulling stressing cables on the corner of a slab were working near a load of trusses that were improperly stored by the employer at residential home under construction, exposing workers to the hazards of being struck by falling trusses.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system:  On or about June 11, 2013, and at times prior thereto, employees performing framing activities were working near a leading edge of a residential construction without the use of fall protection equipment, exposing employees to the hazards of falls greater than 6 feet to the ground.  Rafael Lopez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 315625350, citation number 1, item number 1 and was affirmed as a final order on 08/29/2011, with respect to a workplace located at 2724 Tom Miller Street, Austin, TX 78722.
