29 CFR 1926.1053(b)(1): Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: (a) Septage Building - on or about August 19, 2014 - employees were accessing the attic with a step ladder that did not reach the upper landing area. The upper landing to the attic area was at a 15 foot elevation from the concrete slab. The employees were using a 12 foot fiberglass step ladder to access this area.

29 CFR 1926.1060(b): The employer did not provide retraining as necessary so that each employee exposed to fall hazards maintained the understanding and knowledge acquired through initial training: (a) Septage Building - on or about August 19, 2014 - the employer did not provide ladder retraining to the employees that do not have a clear understanding of the proper set-up and use of portable ladders.
