29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: On or about May 21, 2014, at or near 4232 Bluebonnet Boulevard, Baton Rouge, LA; employees were observed working on a roof with unprotected sides and edges approximately 12 feet above the ground. The employees were not protected from falling by a guardrail system with toeboards or a safety net system. And, the employer did not ensure that employees used their personal fall arrest systems properly.

29 CFR 1926.1053(b)(4): Ladder(s) were used for purposes other than the purpose for which they were designed: On or about May 21, 2014, at or near 4232 Bluebonnet Boulevard, Baton Rouge, LA; employees were observed using a step ladder for a purpose other than which it was designed. Despite being a self-supporting ladder, it was leaned up against the side of the building in order to gain access the roof. And, there were several feet between the top rung of the ladder and the bottom of the roof.
