29 CFR 1910.212(a)(4):     Revolving drum(s), barrel(s), or container(s) were not guarded by enclosure(s) which were interlocked with the drive mechanism so that the barrel(s), drum(s), or container(s) could not revolve unless the enclosure(s) were in place:    (a) Line 5/6 mixing barrel was not adequately guarded to prevent inadvertent contact with the rotating mixing barrel. Barrier guarding installed did not prevent employees working around the rotating barrel mixer from inadvertently coming into contact with the rotating barrel.    (b) Line 8/9 mixing barrel was not adequately guarded to prevent inadvertent contact with the rotating mixing barrel. Barrier guarding installed did not prevent employees working around the rotating barrel mixer from inadvertently coming into contact with the rotating barrel.

29 CFR 1910.303(b)(7)(iv): There were damaged parts that may adversely affect safe operation or mechanical strength of electric equipment, such as parts that were broken, bent, cut or deteriorated by corrosion, chemical action, or overheating:    (a) Breaker Panel - Rail Unloading Area: The Main Breaker panel supplying power to several disconnect boxes and other electrical equipment had a damaged enclosure door. The door did not have a securing mechanism to secure the panel door in a closed position and allow only qualified/authorized personnel to enter the breaker panel.  The breaker panel door securing and locking device was broken out of the door and did not allow the door to be secured and locked in a closed position.    (b)  Main Breaker Panel - Rail Unloading Area: The Main Breaker panel supplying power to several disconnect boxes and other electrical equipment had a damaged enclosure door. The door did not have a securing mechanism to secure the panel door in a closed position and allow only qualified/authorized personnel to enter the breaker panel.   The breaker panel door securing and locking device was broken out of the door and did not allow the door to be secured and locked in a closed position.

(b) Main Breaker Panel - Rail Unloading Area: The Main Breaker panel supplying power to several disconnect boxes and other electrical equipment was not identified with the purpose, voltage, current, wattage or other ratings as necessary.     (c) Breaker Panel - Rail Unloading Area: The 440 volt AC breaker panel supplying power to several disconnect boxes and other electrical equipment was not identified with the purpose, current, wattage or other ratings as necessary.

29 CFR 1910.305(a)(1)(i): Metal raceways, cable trays, cable armor, cable sheath, enclosures, frames, fittings, and noncurrent-carrying parts that are to serve as grounding conductors, with or without the use of supplementary equipment grounding conductors, were not effectively bonded where necessary to ensure electrical continuity and did not have the capacity to conduct safely any fault current likely to have been imposed on them:    (a) Bin #2 Area: Liquidtight Flexible Metal Conduit (LFMC) located to the right of bin #2 conveyor salt shoot was broken away from the connection fitting exposing electrical wiring and exposing employees to an electrical shock hazard.    (b) Ranco Bin D Utility: Liquidtight Flexible Metal Conduit (LFMC) run from 24' blend auger disconnect to the 13-124 motor was broken away from the motor fitting connection exposing the electrical wiring. This exposed employees to an electrical shock hazard.
